Despite AI’s definite contribution to identifying more polyps during colonoscopies, the panel indicated that the technology might only lead to a marginal reduction of two colorectal cancer-related deaths per 10,000 individuals over a decade. This cautious finding stems from what the experts termed ‘fuzzy evidence,’ prompting them to refrain from a definitive recommendation.

The Dilemma of Emerging Technology

This uncertainty highlights the trade-offs associated with implementing new screening technologies. While improved ADR is a vital marker of quality care and correlates with improved health outcomes, the cost implications of more testing and procedures could outweigh potential benefits. Authors of the guideline, such as Benjamin Lebwohl and Shahnaz Sultan, noted that the increase in ADR largely results from detecting small polyps with a low likelihood of progression, raising further questions about meaningful cancer and mortality reduction.

A revision to the guidelines is scheduled in one to two years when improved data may be available.
